Report of Additions by Baptism, Profession of Faith, Reclaim Worksheet
The following worksheet is used when individuals are added to the church through Baptism, Profession of Faith, or being Reclaimed. This document is important because it gives the Church consent to use their personal information for administrative purposes.

FUR (Family Unity Record)
Family Unit Record (FUR) is a document that we use to track information about the families in our churches. It has information about their children, their address, baptismal dates, and membership transfers within eAdventist.
Local Church Statistical Report
This is a report that our Church Clerks submit to the Conference once each quarter. It consists of attendance information obtained on the 2nd Sabbath and 7th Sabbath of each quarter.
Transfer-In Worksheet
When an individual or a family is transferring to a Local Church, the head of the household must fill out, sign, and date a Transfer-In Worksheet Form.
Membership Changes-Transfer Requests-Death-Missing-Removed Form
This is a form that our Church Clerks fill out when an individual requests that their membership be transferred to our (Church Clerk’s) Local Church.
